This Consent Notice (“Notice”) is provided by EarlyFit Health Private Limited (“EarlyFit,” “we,” “us,” or “our”) as part of our commitment to protect the personal data of Users (“User,” “you,” or “your”). You consent to receiving medical consultations, advice, and therapy from EarlyFit's affiliated doctors, nutritionists, physical therapists, and psychologists via telecommunication methods (e.g., video calls, audio calls, chat) through the Platform. You understand that telehealth offers benefits like convenience but has limitations, such as the absence of a physical examination.

4. Understanding Prescription Medications (e.g., GLP-1 Agonists)

You acknowledge that the EarlyFit program is medically supervised and may involve prescription medications (e.g., GLP-1 agonists like tirzepatide, semaglutide, or metformin) if deemed appropriate by an EarlyFit-affiliated doctor after consultation. The doctor is responsible for discussing the medication's benefits, risks, and potential side effects with you. You must provide informed consent directly to the doctor for such treatment and promptly report any side effects to your Care Team.

5. Accuracy of Data You Provide (Especially Orally)

You agree to ensure that personal data shared with EarlyFit staff or your Care Team, particularly during unrecorded phone or video calls, is accurate and complete. EarlyFit relies on this data and is not responsible for issues arising from incorrect or incomplete information provided by you.

6. Allowing EarlyFit to Help with Lab Tests and Medications (One-Time Opt-In)

As part of the EarlyFit program, you provide explicit permission for EarlyFit to act on your behalf for the duration of your program to:

You understand that EarlyFit is not responsible for the accuracy of data from third-party devices or platforms.

8. EarlyFit Is NOT for Emergencies

You understand and agree that the EarlyFit Platform and services are not for medical emergencies. In case of a medical emergency, you must immediately contact local emergency services (e.g., call 108 or 112 in India) or visit the nearest hospital.

9. Your Responsibility for Program Success

You agree to provide truthful, accurate, and complete data, especially about your health, and understand that program success depends on your active participation and commitment to following personalized plans and medical advice from your Care Team.

10. Withdrawal of Consent

You may withdraw your consent for the processing of your personal data at any time by contacting [email protected]. EarlyFit will cease processing your personal data upon withdrawal, but may retain or share data with government authorities as required by applicable law.
